(aka medication chaos)

There's one thing I really have to get off my chest. Yes, documentation is a bottomless pit. Trying to update existing documentation is Pandora's box. But my face. I'd rather have no documentation at all than what we've been working on here since last year.

It's not as if there were correct medi-plans both from me and, congruently, from various of my doctors. But they are no longer used because "the company wants everything to be on their form". This does not correspond to the relevant specifications for medication plans, but who cares?

My staff, if they worked like that....

Hello dear practice team,

Attached, as discussed with my nurse, is the most recent prescription schedule available here. It is a mystery to me how a nursing service can work with this. Thank you for the revision.

The following strikes me at first glance:

    - Antra I only take 20mg 1-0-0-0 as far as I know.

    - With venlaflaxine, I would ask you to check whether it is compatible with mirtazapine. I think I have read something along these lines.

    - The antibiotic, I don't know, I've actually been on one again since yesterday until 6 more days. But after that it would have to be taken off again. Is it worth the effort?

    - Creatine 5g 1-0-0-0 is not an on-demand medication but should be given daily (from the initial prescription after diagnosis in Großhadern, 2018).

    - Morphine sulphate tablets are new to me. So far, I have only had drops and taken 4-6 drops when needed or requested.

    - Please delete B12 effervescent tablets. I had ordered too many of these on Amazon in 2018 and because they were there, my nursing service took it as a request to have them included.

    - If I'm not completely blind, 1x mirtazapine 15mg 0-0-1-0 is missing from the standard medication in the evening for sleeping.

    - Vomax suppositories are also missing when needed.

    - Need: Zolpidem 10mg

    - Need: Zolpidem 5mg

    - Standard: Movicol not 1 every 2 days, but ½ every day

    - Standard: Scopoderm TTS scopolamine patch every 72 hours

    - Standard: Otowaxol for ear cleaning once a week 0-1-0-0

    - Need: nasiq nasal spray for nasal congestion

From today I would add

    - Need: paracetamol max. 3 times a day for fever over 38.5°C

I'm sure I've overlooked something. But who knows, maybe not.

Thank you ?

Kind regards
